Future Centered Care Service Offerings:

Building The Team:

Our focus is to develop and sustain a team of caring, knowledgeable personal and professional support.  The goal is to build the team now while families can enjoy the benefits of this process together, in real time. We identify supports already in place, engage family and friends, and recruit/maintain a team of professionals including life and job coaches, support staff, therapists, psychiatrists, medical doctors, specialists, nutritionists, financial planners, attorneys etc. 

Living Environment:

We make regular visits to the client’s home to assess safety, cleanliness, routine maintenance, supplies, and food. We work on daily living skills such as personal hygiene, daily scheduling, money management, transportation, and time management.

Community Connections:

We build, strengthen, and expand positive social networks. This includes hobbies, recreational activities, and religious affiliations. We facilitate and support ongoing friendships and/or romantic relationships.

Self Advocacy and Supported Decision Making:

We work one on one in the community or in the client’s home. We use role-playing, social stories, and coaching to work on self-advocacy and self-determination. We focus on assisting in safely navigating the individual’s in person and online communities.

Executive Functioning Coaching:

Throughout our Executive Functioning training, we start by focusing on which cognitive abilities may be inhibiting the client’s ability to regulate and manage their executive functions. We use practical examples from their daily lives to better understand how the client thinks about and problem solves different scenarios. We teach clients about how the brain works and specifically about how their brain navigates situations.

Through a series of practical tools, repetition and evaluation, we determine strategies that help clients improve their self awareness, emotional regulation and self-monitoring. The coach and the client meet regularly to assess the effectiveness of the new strategies. We focus on enabling the client to create their own strategies that will work for them. Once established, we work to reinforce the skills and enable them to maintain them as independently as possible.

Executive Functioning Coaching Fees: $250/hr

Parent and Family Coaching:

We work with the family to guide and support them through transitions and decisions that impact the future. Our goal is to facilitate strong communication within the family and team, and coach them through challenges that come up along the way. We recognize the complex role and responsibilities held by the parent/family while valuing a person centered approach. 

Medical Care and Health:

We proactively identify, address, and coordinate healthcare needs so we can prevent and/or minimize a crisis from occurring. We schedule and attend doctors’ appointments, ensure medication is refilled, arrange transportation, and communicate with all medical professionals and relevant team members.

Future Planning:

In partnership with the family and others on the team (i.e. financial planner, attorney), we work to develop and implement a Letter of Intent in conjunction with their customized care plan, which serves as a roadmap for the future. Change and transitions are inevitable, so planning is an ongoing process. We guide the family through future planning steps making connections to and working with experts in each area (i.e., legal, financial, benefits, housing options, and advance care planning).

Benefits Maximization and Self-Direction Support:

We guide the individual and family through the process of navigating and staying current on changes to the Medicaid and OPWDD benefits systems. We manage invoices and reimbursements and focus on helping self-directed families maximize their budgets to take advantage of the funding available to them.

Volunteer, Internships and Employment:

We identify client’s strengths and develop and/or customize volunteer, internship and/or employment opportunities. We build a system of support to ensure these opportunities are successful and sustainable through supervision and support of both job and life coaches. 

Housing Navigation Services:

We work with individuals and families who are interested in moving to community-based housing to obtain and maintain stable, long-term housing of their choice. We work with the individual to develop a person-centered housing plan, which includes their housing vision and the necessary supports needed. We help to identify current options as well as make connections to like-minded adults, families and providers to create shared living opportunities.

Housing Navigation services include: Developing an individual housing action plan; Implementing a housing action plan; Finding a Home; Coordinating a move; Housing sustainability plan and transition to ongoing service providers; Housing crisis resolution.

Housing Navigation Fees: $250/hour

Why Now?


An estimated 80% of adults with intellectual and/or developmental disabilities (I/DD) live with a family caregiver.

NY state is home to 225,000 adults with I/DD who will need some form of staffing and residential support throughout their lifespan.

Approximately 500,000 people in the United States with developmental disabilities will become legal adults over the next decade.
