  • We provide customized case management for neurodiverse adults, tailoring our services to meet each client’s individual needs. Our Case Managers work with clients on a number of areas, including enhancing daily living skills, cultivating and nurturing meaningful friendships and relationships, securing employment opportunities, overseeing healthcare management, maximizing benefits and determining and developing optimal living options. We excel in bridging gaps where systems fall short. Through comprehensive, curated care, we partner with our clients and their families to achieve their goals.

  • Our team is made up of social workers, occupational therapists, employment specialists and beyond. Our work extends to job coaching, counseling, community building, and more. We pride ourselves in being matchmakers - facilitators of connection between people, resources, and services.

  • Clients can expect periodic in-person check-ins with their designated Case Manager. Additionally, proactive ongoing communication will be maintained during the week through the client’s preferred methods - whether it be phone, text message, Zoom and/or email. We are also committed to providing regular updates to key stakeholders (parents, caregivers, etc.) to discuss progress and review any adaptations to the care plan.

  • Yes! We are able to assist individuals and families to enroll in OPWDD services and entitlements, particularly in the realm of Self-Direction (SD). We guide families every step of the way and help launch your Self Direction budgets while working collaboratively with the Care Manager (Care Coordination Organization), and identifying brokers and fiscal intermediaries that will be a good match for the adult and family.

  • Yes! We enable families to maximize their OPWDD Self-Direction budget. Our involvement can also extend to overseeing reimbursements, onboarding staff we recruit through the FI, and managing other administrative aspects to make Self-Direction sustainable.

  • We act as supplemental support and do not take the place of your care manager, broker, or fiscal intermediary (FI). We work alongside your established circle of support and provide additional advocacy and guidance throughout the OPWDD and Self-Direction processes. We also assist families in finding brokers, FIs, and maximizing their budget.

  • The time frame in which we support clients varies - for some this means using our services to meet immediate needs for a specific life transition, whereas others enlist our help for long term/ongoing support needs. Typically families are working with us for at least 9 months to provide time to build a strong relationship and address immediate/intermediate needs.

  • We currently provide services in Westchester County, Nassau County, Long Island and within the five boroughs of New York City. If you are interested but live beyond our current service area, please reach out, and we will keep you informed about our plans for future expansion.

  • Anywhere! Our case managers are flexible and can meet at your home, workplace or your favorite local spot. We want to see our clients in a range of environments interacting with different people and scenarios so we can learn as much about them and their varying strengths. Additionally, you can choose to meet at our shared office space in the West Village of Manhattan.

  • Our current client base spans from 18 to 60+ years old and has a wide spectrum of communication styles, physical abilities and levels of independence. Our clients reside in a range of housing options, including family homes, college residences, certified housing, and self-directed supported housing.

  • FCC is excited to collaborate with your current team, aiming to foster alignment, enhance communication and provide support. We also fill in gaps if additional services or specialists are needed and/or when any shifts occur with key team members (moving, retirement, etc.)

  • Case managers and coaches work collaboratively to support their clients. At times, clients benefit from additional support that can be provided by our coaches. This could look like help with routine travel to and from work, practicing a specific skill until able to do so independently or having support during social and recreational programming. Case Managers will vet, recruit, and provide ongoing supervision to the assigned coaches. They also assist with the onboarding process with the Fiscal Intermediary for those who have Self-Direction.

  • During the first month of working with FCC, clients and families are supported through a structured onboarding process guided by an initial care proposal. Case Managers meet regularly with the client and coordinate weekly calls with the family when needed. Initial priorities of the Case Manager include getting to know the client, their family and their current support system. Together, they identify any potential gaps in services, schedules or benefits in order to create a clear path forward. The care plan is then adapted and bolstered to serve as an evolving roadmap for FCC’s ongoing partnership with the client, their family and team.

  • Our services are typically paid out of pocket by families on a monthly retainer basis. While we have been able to work with some insurance companies and certain Fiscal Intermediaries for reimbursement, it has covered only a small percentage of our fees. If you have any specific questions about our retainer structure or payment options, please feel free to contact us directly. We continue to advocate to OPWDD for the need to cover the type of services we provide to enable Self-Direction to be more sustainable.
